Progress notes should serve two goals: clinical continuity and billing defensibility. When either side is weak, clinics feel the impact.

Minimum note structure

  1. Subjective: patient-reported pain, functional changes, home exercise adherence.
  2. Objective: measurable ROM, strength, gait, balance, functional test results.
  3. Assessment: response to treatment and progress toward goals.
  4. Plan: next interventions and session frequency adjustments.

Consistent structure makes handoffs cleaner and payer reviews faster.
